FEES & INSURANCE

Therapy is an investment in your well-being, and understanding the financial side of care should feel as straightforward as possible.

Mending Forge Therapy & Wellness Center, LLC offers several payment options, including in-network insurance through Headway, out-of-network benefits, and private pay.

Option 1: A Insurance Through Headway

Mending Forge Therapy & Wellness Center, LLC is a private-pay practice and is not directly contracted with insurance companies.

I am individually credentialed with participating insurance plans through Headway, which provides a separate option for eligible clients who would like to use their in-network insurance benefits for therapy with me.

I currently participate through Headway with:

Aetna • Ascension • Carelon Behavioral Health • Cigna

When you choose to use insurance, eligibility, billing, and insurance claims are managed through Headway. Your individual cost will depend on your specific plan and benefits. Verification of benefits or an estimated cost does not guarantee coverage or payment.

Option 2: Out-of-Network Benefits

If your insurance plan includes out-of-network mental health benefits, you may be eligible for reimbursement for a portion of the cost of therapy provided through Mending Forge Therapy & Wellness Center LLC.

Your reimbursement and out-of-pocket responsibility will depend on your individual insurance plan, deductible, and benefits.

Option 3: Private Pay

You are always welcome to choose private-pay services through Mending Forge Therapy & Wellness Center, LLC.

Why Choose Private Pay?

Private pay can be a good fit for clients who prefer to receive therapy outside of the insurance process.

Some potential benefits include:

  • More privacy from insurance involvement. When you choose private pay and do not submit claims for reimbursement, your treatment information does not need to be shared with an insurance company for claims processing.
  • With private pay, insurance reimbursement requirements do not determine whether you can receive services.
  • Greater flexibility in your care. Treatment decisions can be based on your individual needs and clinical recommendations rather than an insurance company's requirements for covered services.
  • No insurance authorization or medical-necessity reviews. Private-pay services are not dependent upon an insurer authorizing treatment or determining whether continued treatment meets its requirements for reimbursement.
  • Clear, predictable fees. You know the private-pay cost of your session without having to navigate changing deductibles, copays, coinsurance, or claim determinations.

Choosing private pay or using insurance is a personal decision. My goal is to provide clear information about your options so you can choose the payment pathway that works best for you.

Private pay services fees are listed below so you know what to expect before scheduling.

Insurance & Reimbursement Disclaimer

Insurance coverage and benefits vary by plan and are determined by the client's insurance carrier. Mending Forge Therapy & Wellness Center, LLC and the provider cannot guarantee eligibility, coverage, reimbursement, claim approval, or the amount an insurance plan will pay for services.

For clients using in-network benefits through Headway, eligibility, benefit estimates, claims processing, and insurance billing are managed through Headway and the participating insurance plan. Copays, coinsurance, deductibles, and other financial responsibility are determined by the client's individual plan.

For clients choosing private pay and seeking out-of-network reimbursement, a superbill may be provided upon request. A superbill does not guarantee reimbursement. Acceptance of the superbill, determination of out-of-network benefits, and the amount of any reimbursement are determined solely by the client's insurance plan.

Verification of benefits, preauthorization, or an estimate of cost is not a guarantee of coverage or payment. Insurance benefits and claim determinations may change or differ from initial estimates.

Clients are encouraged to contact their insurance carrier or applicable billing platform directly with questions regarding coverage, benefits, deductibles, reimbursement, or financial responsibility. Clients remain responsible for charges not covered or reimbursed by their insurance plan in accordance with the applicable financial agreement and practice policies.
