Cancellation Fee

If you have not provided a 24-hour cancellation notice, you will be responsible for the full fee for the appointment scheduled. 

 

Communication

Communication responding to phone calls, emails, and text messages that last longer than 10 minutes will be prorated per minute based upon the provider’s defined fees, which are listed as follows:

Court Involvement  

The Mending Forge Therapy & Wellness Center requires that a contract for services rendered be signed before engagement in court proceedings which includes record review, court preparation, travel time, communication with legal counsel and other parties privy to the case in question including travel time. 

 

• A $2,000 retainer is due upfront, and the hourly fee will be applied to the retainer monies.

• The hourly fee for all court involvement for Edith Appleton, LCSW is $250 per hour. 

• Should additional retainer monies be needed, therapist Edith Appleton will communicate this in writing.

• ​If the court date is cancelled before scheduled testimony, fees will not be reimbursed. This is due to the therapist clearing off their schedule to see clients to attend court.
