Fees & Insurance

The Mending Forge Therapy & Wellness Center is considered private pay and does not take insurance. As a private-pay center, our service fees are due upon service.  Please know that we are happy to provide you with a superbill that you can submit to your insurance company for reimbursement of services. Please note that receiving a super bill to submit to insurance does not guarantee that you will be reimbursed for services you pay out of pocket for.

Cancellation Fee
If you have not provided a 24-hour cancellation notice, you will be responsible for the full fee for the scheduled appointment. 
 
Communication
Communication responding to phone calls, emails, and text messages that last longer than 10 minutes will be prorated per minute based upon the provider’s defined fees, which are listed as follows: 
Graduate Intern Rate Per Minute-2. 49
Clinical Director Per Minute-5.49
 
Court Involvement  
The Mending Forge Therapy & Wellness Center requires that a contract for services rendered be signed before engaging in court proceedings, which includes record review, court preparation, communication with legal counsel and other parties privy to the case, and travel time. 
 
  • A $2,000 retainer is due upfront, and the hourly fee will be applied to the retainer monies.
  • The hourly fee for all court involvement for Edith Appleton, LCSW is $250 per hour. 
  • Should additional retainer monies be needed, therapist Edith Appleton will communicate this in writing.
  • ​If the court date is cancelled before scheduled testimony, fees will not be reimbursed. This is due to the therapist clearing off their schedule to see clients to attend court.
